Onboarding 101: Paying with a Credit/Debit Card

The following instructions are a continuation of steps 1-6 from Onboarding 101: I want to bring Emily's Hope to my District/School. 

If you are NOT exempt from sales tax: You will not see the option to enter your credit card information. Please continue filling out this form. We will confirm if you owe any sales tax and will follow up with a revised total in 2-5 business days. You will not be able to pay today. 

7. Enter your credit or debit card information and click Next. Your card won't be charged until you finish the entire enrollment form.
8. Enter your shipping and billing addresses.
9. You will be asked a few questions about where you heard about the curriculum, and you have the opportunity to write any notes for our team. 

10. You must accept the terms and conditions for the use of the Emily's Hope Substance Use Prevention Curriculum. 

    If you are unable to accept any part of the terms or conditions, please contact our team. You can choose to check that you accept, and we will fix it after you email us, or click 'Save' to ensure you can go back and finish your enrollment form. 

11. You will receive an email from noreply@jotform.com with a receipt attached and your enrollment ID.
12. To access lessons, you will need to enroll your educators in the Teacher Hub using this form and your enrollment ID, which can be found in your email. Fill out the form for each educator who will be teaching the curriculum. 

If YOU are also teaching the curriculum, you will also need to submit your email address to this form.

13. The next steps depend on your tax-exempt status and whether you uploaded a certificate during the onboarding process:
    1. If you ARE tax-exempt and you uploaded your tax-exemption certificate, you will receive access to the Teacher Hub within a few minutes of filling out this form. We will also begin preparing your shipment within 2-5 business days of any physical materials.
    2. If you ARE tax-exempt, but did not upload your tax-exemption certificate during the onboarding process, please email it to bookorders@emilyshopeedu.org. We cannot provide access or ship materials until we receive the necessary information.
    3. If you ARE NOT tax-exempt, we will confirm if you owe any sales tax and will follow up with a revised total in 2-5 business days. After you pay, you will receive access to the Teacher Hub.
14. Now it's time to get your educators started! You can send this help article to your staff, or follow it yourself if you will be teaching the curriculum.
